GPU core clock fluctuations and instabilities

Question

So my GPU has always been overclocked but recently I noticed games running just a bit slower, no stuttering or anything to speak of. In Afterburner it shows my core clock constantly fluctuating, not in the throttling to idle way but small, random and sporadic changes. Even after going back to completely default settings with no overclock it still fluctuates. However this is fixed when I up the power limit to +20% but allows not even a 10MHZ overclock without the fluctuations occurring and effectively giving me no extra performance.

This isn't terrible as the card can still run fine at base speeds but is a bit concerning. Not to mention any more than 1.15v on the core crashes it. COuld this be a sign my gpu is on its way out?

System:

Windows 10 Pro (version: 1511, build: 10586.318)

Asus DCU2 AMD 7950 v1 (which have a lot of cooling issues) (driver version: 16.3.2)

i5-2500k

12gb RAM
